Here is a repeat of the Embassy information for all those who have 
said they didn't get it before. Please bring photocopies of all 
important documents as well as the originals. There will be a 
photographer there to take passport photos for your convenience.

(16 and older)
Item 1 Complete Passport Application Form If you are renewing a
ten-year
passport that has not yet expired, or has expired within the last five
years, and you are sill have the passport, Complete DS- 82 (red
form)Otherwise, complete form DSP-11 (brown form)

Item 2 Provide Proof of Identity Photo ID:  Passport, Teudat Zehut,
Driver's License

Item 3 Provide Proof of Citizenship U.S. passport, Certificate of
Naturalization, U.S Birth Certificate, Consular Report of Birth
Abroad,
Certificate of Citizenship

Item 4 Provide Two Photos 5 cm x 5 cm, on white background

Item 5 Fee:DSP-82 (Red Form)               -  $67 (cash US dollars or
shekel equivalent)DSP- 11 (Brown Form)      -     $97 (cash US collars
or
shekel equivalent)
Document Return Policy:Please, provide us with a
self-addressed, stamped envelope (bring at least 10 NIS in postage)
and we
will be glad to return your new passport by registered mail. Or, we
will
send it by Moniot  Aviv, courier service for NIS 45 cash-on -delivery
to
your home or office.
