Here is a repeat of the information for those who didn't see it the 
first time. If you already signed up and can't come please tell me as 
soon as possible. There will be a photographer there to take passport 
photos, for your convenience. Please bring photocopies of all 
documents as well as the originals.

Fee $ 97 (cash) US dollars or shekelEquivalent
Document Return PolicyPlease provide us with a stamped,
self-addressed Envelope (bring at least 10 NIS in postage).  We
willreturn
your new passport by registered mail.  Or, we will send it by Monit
Aviv,
courier service for NIS 45 cash-on-delivery to your home or office.

FOR CHILDREN UNDER 14, THE FOLLOWING IS REQUIRED:

Item 6 Provide Proof of Parentage: Original Birth Certificate of

stating both parents.

Item 8 Provide Permission from Both Parents: Both parents must be
